Custom Fit Scleral Contact Lenses

Case report assisted and written by Wendi Zhang from the Illinois College of Optometry. Presumed graduation date of May 2025.

A 39-year-old male presented to our clinic after learning about scleral lenses online. The patient had been diagnosed with keratoconus twenty years ago and for the past several years had been wearing an RGP lens in the right eye.

Patient had previously been fit with a hybrid lens in the left eye, but did not wear the lens due to significant discomfort. Entrance acuities were 20/20 OD and CF OS, no visual improvement with refraction. Topography showed significant steepening in both eyes with Kmax 64.3D OD and 102.0D OS.

The patient was fit with custom scleral lenses in both eyes, achieving a BCVA of 20/20 OD and 20/50 OS, as well as noting significant improvement in comfort of the lenses.

We are currently looking at adding higher order aberration (HOA) correction to the left lens in the hopes of further improving visual acuity.

Higher order aberrations are caused by small imperfections in the cornea that can’t be corrected with regular lenses, but new technology is making it possible to identify and correct them.
