Dry Eye Disease Affecting on Vision

New 55 year old woman presents for a medical consultation for dry eye OU. Entering visual acuities were 20/200 OD and 20/30 OS through habitual SRx.

With refraction, the patient improved to 20/80 OD and 20/30 OS. All entrance testing was WNL. Slit lamp exam revealed 3+ SPK OD (image 1) with all other findings being WNL.

Patient had corneal sensitivity OU. After discussing with the patient, it was decided that scleral lenses would be advantageous over any SRx.

The patient was fit into Zenlens sclerals in office through which she was able to achieve visual acuities of 20/50 OD and 20/30 OS. While waiting for her custom scleral lenses to come in, an amniotic membrane with a bandage contact lens was placed in office OD.

Patient was also started on moxifloxacin QID OD. Patient was followed up numerous times appropriately for the amniotic membrane and BCL removal and to monitor the status of her improving SPK (Image 2).

At the scleral lens dispense visit, with a small over refraction, the patient was able to achieve visual acuities of 20/40+ OD and 20/25 OS. The patient was thrilled with her vision and comfort.
