Long Standing Keratoconus

A 52 year old new male patient presents for a scleral lens fitting for longstanding KCN.

Pt. is a longstanding scleral lens user as well but had just recently moved to Minnesota and was in need of a new provider who is comfortable fitting these lenses.

He was mostly happy with his lenses but complained of a general redness and irritation of his lens wearing in the Right eye. This patient is monocular as his left eye recently went totally blind due to radiation for a melanoma near his eyelids and now wears a patch OS.

Pt. presented with VA of 20/50-1 OD and had significant redness of his conjunctiva. A pentacam scan confirmed the diagnosis of KCN and a slit lamp examination showed (as seen in the photos) that his current lens was extremely tight and adjustments were necessary for optimum comfort.

A scleral lens was placed on his eye in office which was able to achieve 20/25 vision after an over-refraction. He returned in 2 weeks to pick up his newly ordered lens and was instantly relieved upon insertion.

He expressed a large improvement in comfort and vision in his only functional eye which was supported by 20/20 vision and significantly less blanching of his conjunctival vessels.

It was revealed that he had been dealing with the previous lens’ parameters for 4 years, thinking that his comfort and vision were as good as they could be. He is now very excited to be in something that could improve his life even further.
